Mandatory submission requirements:
- Completion of the Safety Excellence Award Nomination Form
- o Submission of this application is considered consent to allow VICA to access your company's safety rating via WorkSafeBC
By submitting this application, you authorize the Workers' Compensation Board to release the required information pertaining to our Claims Experience record to the Vancouver Island Construction Association for the purpose of establishing our ranking for the Safety Award. I certify that the information contained in this nomination package is true to the best of my knowledge and that the nominated company is a VICA member. The nominee also consents to have their name and photo published in connection with the Safety Excellence Awards.
